Anna apple tree

This apple tree has taken a turn for the worst. What can I do to get it back to health? Also, what is that on the leaf in the second photo? What do I do about it?

Hello I was wondering how your Apple tree is going?
My tropical Anna is looking a bit sick too, although it is quite large and been in for two years.
We recently received a lot of rain, but it survived last Februarys heavy rain. I also recently gave it some fertiliser and mulch and I am now learning about how this can cause issues if over done.
Hope you have managed to fix the issue.
