diseased apple tree

Kylie starts with ...
Can someone please tell me what is wrong with my apple tree and what to do to it. We would hate to lose it. We live near Geelong, Victoria. The tree has also sent up growth from the root stock (I gather it is grafted) but I trimmed this off. Thankyou
